REGISTRATION FLOW
  • You are required to download and print out the application form as printed copies are accepted by TM.
  • After approaching customer, please get customer's address to check for UNIFI coverage. 
  • For order that is out of UNIFI coverage area, you have to wait for TM to release the port. 
  • Personal (Residential) application form is solely for personal package whereas business application form is solely for business package, please do not mix up your submitted forms.
  • After filling up all relevant information, partners are required to scan & email all supporting documents to us.
  • We will process your order only if we receive all supporting documents.
  • Your order will be rejected if you fail to provide all supporting documents.

UNIFI INSTALLATION
  • Installation appointment date would normally be 2 to 4 weeks after registration. However, when we process your UNIFI order into TM portal, we will select the earliest installation date for your customer.
  • Installation appointment date will be known right after UNIFI registration. 
  • Contractor will normally call your customer to confirm the appointment date 2 or 3 days before. Please remind your customer to answer phone call within these period even though the caller is an unknown number. Contractor might be using unknown number to reach your customer.
  • If you want to re-arrange your appointment, kindly contact TM Unifi Center (1-300-88-1221).

REMEMBER TO INFORM YOUR CUSTOMER
  • Deferment of Unifi installation within 48 hours before the appointment date will lead to a penalty of RM200.
  • Standard installation of RM200 (15 meters of fibre optic cable) is waived until further notice. Additional charges will be implied if the fiber optic cabling exceeds 15 meters. It normally happens on landed properties. Your customer has to deal with the contractor personally.
  • For landed properties such as double story, semi-D, bungalow and others, installation is allowed to be done on any day includes weekend.
  • For high rise building with management team such as condo and apartment, installation is only allowed to be done on weekday as most building management team are closed on weekend.

    • A clear photocopy IC of customer for Malaysian or a clear photocopy passport of customer for foreigner (Passport expiry date must not be less than 2 years from registration date)
  • Perniagaan/Business Package - Sole Proprietor (ROB)
    • Application Form with Company Stamp
    • NRIC / Passport
    • SSM ROB - Form D or Form E
  • Perniagaan/Business Package - Sdn Bhd (ROC)
    • Application Form with Company Stamp
    • SSM ROC - Forms 9, 24 & 49
    • NRIC/Passport
  • Perniagaan/Business Package - Berhad (ROC)
    • Application Form with Company Stamp
    • Form 8 & 23
    • Authorized Person Form / Letter and NRIC/Passport
  • Perniagaan/Business Package - Foreign Company
    • Application Form with Company Stamp
    • Form 79, 80/80A, 83/83A
